Application Form For Admission in Level II/III Program in Medical Clinical Specialties Leading to Qualifications of FCPS/MD/MS/MCPS Programs

Eligibility / Pre-Qualifications

The following Eligibility / Pre-Qualifications criteria should be met in order to apply for admission through Central Admission Policy

  • MBBS Degree.
  • Certificate of Completion of House Job.
  • PM&DC Registration.
  • Result of passing Part-I Examination of FCPS and Entrance Test for MD/MS/Diploma within the last three (03) year.

ATTACHMENTS 

Candidate will need to submit PDF Scans of the following original documents. 

  1. Both Sides of a valid NID Card.
  2. Local / Domicile Certificate.
  3. MBBS Degree.
  4. A clear, uncut copy, with the authorized signatory stamp and signature, of one of the following
    1. Official Transcript of Records.
    2. Consolidated Marksheet.
    3. All Year Mark Sheets (Separate each year) 
  5. A Certificate by University/Principal mentioning attempts of all university examinations.
  6. House Job Certificate. 
  7. Valid PM&DC Registration Certification. 
  8. CPSP Part-I Result Card / PGMI entrance Test result (within last 03 years). 
  9. Job Experience Certificate in Public primary and secondary (Non-Teaching Hospitals) and tertiary Healthcare facilities. Shortage of even one day is not allowed to complete a year. 
  10. Published Papers (up to 02 papers) in National and International Impact factor Journals  will be accepted.
  11. The Foreign Graduates must also submit a PDF Scan of the PM&DC Equivalence Examination Result Card/Equivalence Certificate.
